Deceased was found dead in the screen room of the washery. His jacket had been caught by the keyway in the projecting end
of the shaft of the elevator pulley, and was wound round the shaft until his arm and head were drawn up quite tightly
against it and his neck dislocated.

Deceased was assisting to get a load off a gravity hoist. The man at the brake left it thoughtlessly to assist also. When the
load came off, the empty cage shot up and deceased's head was caught between the bottom ring of the cage and a girder
forming part of the framework of the hoist. An empty tub had been left on the top cage.

Deceased and three others were standing in the stoke-hole in front of a range of boilers, when the wall which formed the front
of the slack-hopper suddenly collapsed and fell in upon them along a length of 57 feet at top and 35 feet at bottom. Deceased
who stood close to the wall was buried by the bricks and slack, and killed instantly. The wall was 7 feet 9 inches in
height, built without batter or cross-ties of any kind in the length which fell. It was 14 inches in thickness for a height
of 4 feet, and 9 inches in thickness for the remaining 3 feet 9 inches. The hopper was not quite so full as usual; when the
accident happened. The wall gave no warning, and showed no signs of bulging before it gave way.

SUPPLEMENT TO APPENDIX I.

ACCIDENTS AND FATALITIES NOT COMPRISED IN THE LISTS.

The following accidents and fatalities were reported or came to my knowledge but are not considered to come within the scope of the Mines or Quarries Acts, and are not classed as mining or quarry accidents.

On 9th January, a publican jumped off a bridge between Nos. 4 and 18 pit banks, at Birchenwood Colliery, and was killed.

On 17th January, a man was crushed by a locomotive at Holly Hall Bridge while trespassing on a private branch railway.

On 18th January, a man committed suicide by throwing himself down a water pit at Himley No. 19. He had been out of employment and in a depressed state.

On 10th February, a roadman was taken ill in Park Hall Pit, Longton, and died on the following day from ulcer on stomach. No inquest was held.

On 3rd March, a timberman died suddenly on pit bank at Dudley Wood, after having been round the pit with the examiner. The coroner's jury found that death was due to natural causes.

On 31st March, a stallman died from the effects of spinal fracture received on 10th January, 1900, in Ashmore Park Colliery from a fall of roof.

On 4th April, a driver, who stated that he had been injured while lifting a tub on the rails in Conduit Colliery, on 11th August, 1904, died as the result of spinal paralysis.

On 8th June, a collier was found dead at his working face in Victoria Pit, Biddulph Valley, with his lamp, still burning, hanging near him. Owing to the removal of the body to deceased's home in Cheshire, serious delay occurred before a post mortem was made and the inquest held. The doctor found no external signs of violence, the internal organs appeared to have been healthy, but owing to putrefaction he was unable to ascertain definitely the cause of death.

The death of a contractor on 31st July, owing it was alleged to his having sprained his neck at Sneyd Colliery on 11th June, was found by the coroner's jury to have been due to natural causes.

On 16th July, a pikeman died, presumably from injuries received by a fall of side in Old Ettingshall Colliery, in January, 1904. No inquest was held.

On 1st November, a labourer died from injuries received at Rough Hay old pumping pit, on same day, owing to having been struck on the head by a winch handle while assisting to take down some old machinery.

On 29th November, a joiner became ill while at work, in the workshop at Himley Colliery. He died on 3rd December, the coroner's jury finding that death was due to natural causes.

On 4th December, a woman, not employed, was injured while picking coal at the dirt heap at Brettell Lane Colliery. On 19th December, an old man, not employed, but permitted by the owner to pick coal on a tramway at Foxyards Pit, was run over and killed.

On 24th January, a man, not employed, was killed by falling into Tunnel Quarry, Essex.
